INSURANCE

BCBS

We accept all BCBS policies and provide a complimentary eligibility check before your first session. Every client is encouraged to contact their insurance company to verify their deductible, copay/coninsurance and coverage. If your policy does not cover out patient mental health services, payment will be due within 7 days upon receipt of insurance remit.

OTHER INSURANCES

We can provide clients with a Super Bill to submit to your insurance company for reimbursement. All appointments must be paid in full at the time service in order to receive a Super Bill.

 

Our clinic cannot guarantee any reimbursements. 

PAYMENTS

Our clinic accepts cash, check, HSA and all major credit cards.

CANCELLATION FEE

Our clinicians have dedicated your designated time slot just for you. If you cannot make your appointment, please call, leave a voicemail or email admin@wellnesscentersusa.com 24 hours before your appointment to avoid a cancellation fee. Any late cancellations or no shows will be charged $35 for the first missed appointment, second offense $70 and the full amount of the self pay fee on the third missed appointment. After one year of no late cancellations or missed appointments, your account will start over with the cancellation fee structure. 

In order to reserve an appointment, all patients must have a valid credit card on file for our cancellation policy and for our office to process payments for services rendered.
